The diagnosis of an illness and disability can be debilitating and cause the patients and their families to have various extents of physical distress, psychological and emotional turmoil as well as social problems arising from the illness. The Medical Social Service endeavours to provide high quality social work services that support the patients and their families to cope with the adversities arising from the illness and contribute to the attainment of holistic patient care through multi-disciplinary collaboration, resource arrangement and community partnership in the hope of ensuring safe discharge and promotion of psychosocial well- being of the patients upon returning to the community.
 
Our team works closely with the clinical teams as well as other allied health professionals in the hope of enhancing patients' rehabilitation. We strive to enhance patients’ acceptance of illness through comprehensive psychosocial assessment, strengthening patients' positive coping towards the illness and formulation of safe discharge plan. We also serve as an important bridge between patients and other health care professionals to facilitate communication in the rehabilitation progress. The Medical Social Workers also made dedicated efforts to coordinate with different stakeholders in the patient’s care plan arrangement so as to ensure the continuity of care through arrangement of suitable resources and collaboration with the community partners when patients are returning to the community.
 
Our services are delivered mainly through counselling services, patient support groups as well as various forms of patient engagement and activities in order to attain the goal of promoting psychosocial well-being of the patients and to enhance community reintegration.
  • Conduct comprehensive psychosocial assessment and identify areas of patient/ family needs for formulation of care plan.
  • Provision of counseling services to enhance patient adjustment and coping in the face of the adversities arising from their illness and disabilities.
  • Provision of carer support services to strengthen the family support to the patients.  
  • Management of acute crisis and practical issues during patient hospitalization.
  • Multi- disciplinary collaboration to achieve integrated patient care and to improve rehabilitation outcomes.
  • Formulation of pre-discharge planning to ensure safe patient discharge.
  • Bridging the needy patients and families with corresponding community resources to facilitate community integration.
